Our Liverpool clinic makes it simpler to use your NDIS plan. Instead of travelling between separate providers, you can see multiple practitioners in one location. Your Physiotherapist can speak directly with your Occupational Therapist or Speech Pathologist about your progress, keeping your care coordinated without extra appointments.
We work with self-managed, plan-managed, and NDIA-managed participants. Your support coordinator can refer you directly, or you can contact us yourself. We handle NDIS paperwork including service agreements, progress reports, and plan review documentation.
We carry out assessments, create therapy plans, recommend assistive technology, and write reports that meet NDIS requirements. Your funding stays focused on the goals that matter to you.

We also offer mobile appointments and telehealth for those who cannot travel to the clinic.
Where can I park at the Liverpool clinic?
Free on-site parking is available in reserved bays at 5/33 Heathcote Road, Moorebank. Accessible parking is near the clinic entrance. If those bays are full, additional street parking is available on Heathcote Road.
Is the clinic accessible?
Our Liverpool clinic has step-free entry, a lift, and accessible bathrooms. The building has wheelchair access throughout. If you have specific access needs, please let us know when booking so we can make sure everything is ready for you.
Do I need a referral?
No referral is needed for NDIS-funded or private appointments. For Medicare-funded sessions, you need a referral from your GP under a Chronic Disease Management plan, which covers up to five allied health sessions per calendar year. DVA Gold Card holders can self-refer. If you are not sure which funding pathway suits you, call us on 02 8599 6281 and we can help you work it out.
How do I use my NDIS plan at this clinic?
You can use your NDIS funding for any of our seven services. We work with self-managed, plan-managed, and NDIA-managed participants. Our NDIS provider number is 4050008640. We handle service agreements, progress reports, and plan review documentation. Your support coordinator can refer you directly, or you can contact us yourself.
What services are available at our Liverpool clinic?
Our Moorebank clinic offers seven disciplines: Physiotherapy, Occupational Therapy, Speech Pathology, Exercise Physiology, Positive Behaviour Support, Podiatry, and Dietetics. All services are available for NDIS participants, Medicare patients, DVA veterans, WorkCover clients, and private clients.
How soon can I get an appointment?
Appointment availability varies by discipline and practitioner. We usually have same-week availability for most services. For urgent needs, call the clinic on 02 8599 6281 and we will do our best to fit you in. You can also book online through our bookings page.
Do you offer telehealth from Liverpool?
Yes. Most services are available via telehealth. We confirm whether telehealth suits your needs during your initial consultation. Telehealth is available for NDIS, Medicare, DVA, and private clients.
What should I bring to my first appointment?
Bring any referrals, scan results, or reports from other providers. If you are an NDIS participant, bring your NDIS plan or plan number. For Medicare patients, bring your GP referral and Medicare card. For DVA, bring your Gold or White Card. Wear comfortable clothing you can move in if you are attending a Physiotherapy or Exercise Physiology session.
Can I see multiple practitioners at the same clinic?
Yes. With seven disciplines in one location, your care team can coordinate without referrals between separate practices. If your Occupational Therapist identifies a need for Speech Pathology input, they can arrange it within our team. This saves you time and keeps your care connected.
