Clinical Care Close to home.

Community nursing brings clinical care out of the hospital and into the places people actually live: at home, in a clinic, or by telehealth. Our AHPRA-registered nurses screen for health risks and write the care plans support workers follow. They train the people providing daily support, review how a condition is tracking, and build one accurate medication list. 

Optimum Health Solutions covers the Illawarra, the South Coast and South West Sydney, with clinics at Nowra and Liverpool. One referral can start a whole cycle of support.

How can I fund Nursing access at Optimum Health Solutions?

Nursing supports may be funded through an NDIS plan, depending on how that plan is set up. Providers can also engage us directly, which is common for training and documentation work that sits with the organisation rather than one participant.

Plan-managed, self-managed and agency-managed participants can all access our nurses. We handle the service agreements and the reporting.

What Do Our Nurses Help With?

Direct Clinical
Care

You come home from hospital and the dressing changes are suddenly your job. Our nurses can come to you and do them, and tell your doctor if anything looks like it is going the wrong way.

Chronic Disease Management

Most days you are the one managing it. Our nurses check in between your doctor visits and go over the bits you are unsure about,
however many times you need to ask.

NURSE-LED SCREENING

When this would be relevant: you notice that something has changed and nobody is sure what it is yet.

An Optimum Health Solutions nurse works through evidence-based screening tools with you, records what they find, and connects you with the right practitioner.

This is offered within clinic, at home, in the community, or delivered via telehealth.

NURSING BUILT SUPPORT PLAN

For people with chronic and complex health needs, a nurse reviews and documents the supports that have already been identified and established, then develops a care plan that the team can follow. The plan outlines the person’s care requirements, the supports in place, roles and responsibilities, and actions to take if the person’s condition changes. This ensures care is delivered consistently, safely, and in accordance with existing clinical recommendations.

High Intensity Skills Training

Optimum nurses offer training for support workers, carers and participants in high intensity daily personal activities, with formal competency sign-off.

Enteral feeding, epilepsy and seizure support, catheter support, complex bowel care, subcutaneous injections.

We also offer training on management plans for asthma and anaphylaxis too.

Clinical Review and Monitoring

Regular eyes on a health condition by someone who knows what they are looking at. Post-hospital reviews, observations, and checking that a care plan is working the way it was written. In clinic or at home.

Medication Reconciliation
and Support

One accurate medication list, built from every source, put in front of the people who use it. For anyone on several medicines, after a hospital stay, or where a regulated restrictive practice is in place.

When a screen points somewhere, we pass the findings on directly, so nobody has to start from scratch or explain their story to someone new.
 
Training can also be built around what your team already has in place. If your support workers are trained in three of the five things they need, we will do the two, not all five.

Frequently Asked Questions

No. You can contact us directly. If you are an NDIS participant, a provider or Support Coordinator can also make the referral on your behalf, and often does.
Yes. Our nurses are AHPRA-registered and work within their scope of practice. Where a service calls for a Registered Nurse specifically, a Registered Nurse does it.
Yes. Training, documentation review and competency renewals are often organisational work rather than participant-funded work. We can invoice the provider directly.
