Private health funds provide rebates for all Exercise Physiology and Physiotherapy services under extras cover.
Referrals from GP’s for Enhanced Primary Care (EPC) Plans and Diabetes Health Plans are accepted.
WorkCover referrals from GP’s or case managers, specialists, rehab providers and allied health providers are accepted.
All veterans are entitled to Exercise Physiology, Physiotherapy, OT & Dietetic services through DVA as a holder of a white or gold card. A D904 form is required.
