What are gaze stability exercises?

Gaze stability exercises are a set of movements a Physiotherapist uses to sharpen the link between your head and your eyes. The goal is steady, clear vision while your head moves.

They sit within vestibular rehabilitation, the area of Physiotherapy that helps people with dizziness, vertigo and balance problems. Research supports them as part of that care, particularly for people whose inner-ear balance signal on one side has been reduced.

How the vestibulo-ocular reflex works

The VOR is what keeps the world still while your head moves. When your head turns one way, your eyes move an equal amount the other way, at the same time, so your gaze stays locked on what you are looking at.

When the VOR is working well, that happens automatically and your vision stays clear. When it is not, your eyes lag behind the head and have to catch up with a fast corrective flick. That lag is what many people feel as blur, dizziness or a sense that the world is moving when it should be still.

Gaze stability exercises tap into something the balance system can do, called adaptation. Given the right practice, it sharpens the timing of the eye movements until they keep pace with your head again.

Signs gaze stability exercises might help

These symptoms have many possible causes, so they are a reason to get assessed rather than a diagnosis on their own. People who benefit often notice:

  • Vision that blurs when they move or turn their head
  • Dizziness brought on by head movement
  • Nausea that comes with movement
  • Feeling unsteady or poorly coordinated when their head is moving

What gaze stability exercises involve

The exercises vary from person to person, because the balance system retunes in a way that is specific to how it is challenged. Most follow the same pattern: you move your head in a set direction while keeping your eyes fixed on a target, either a still one or one moving the opposite way to your head.

A Physiotherapist may also add what are called substitution strategies, worked out with you and matched to your abilities. These teach you to lead with your eyes just before you move your head, so your gaze gets where it is going even while the reflex is still catching up.

The exercises are graded and built around you, so they are worth starting with a Physiotherapist rather than copying from a video. Done at the wrong level, they can stir up symptoms; done at the right level, they give the balance system the steady challenge it needs to adapt.

What a gaze stability assessment involves

Before any exercises, a Physiotherapist assesses how your eyes and balance system work together. That can include checking how clearly you can read while you move your head, and watching for the catch-up eye flicks that point to a lagging reflex. The assessment is what sets the starting level and direction for your exercises, so the plan fits your system rather than a generic template.
