Optimum Health Solutions writes functional capacity assessments, assistive technology and home modification reports, manual handling plans, home and living assessments for Supported Independent Living and Specialist Disability Accommodation, motor proficiency testing, speech and language assessments, swallowing and mealtime plans, communication device recommendations, and the full behaviour support plan cycle.
Our Occupational Therapy, Speech Pathology and Behaviour Support teams sit in one organisation across our clinics in Sydney, regional New South Wales and Tasmania. Referrals are welcome from Support Coordinators, Plan Managers and general practitioners.

Complex assessments and reports
A complex functional capacity assessment involves multiple diagnoses, an advocate in the room, or significant behaviour support needs. We scope it separately, with more assessment time and more contact with the people around the participant built in.
High cost assistive technology above ten thousand dollars is written on the NDIA template, with equipment trials, comparisons, and setup, training and maintenance costed into the request. Complex home modifications are assessed against the NDIA complex modification template.
Where more than one discipline contributes to a report, each contributing clinician is named in it.

Functional Capacity Assessments
This is the evidence a delegate reads when deciding what a plan funds. We assess mobility, cognition, communication and daily living in the home, then write it up against the reasonable and necessary criteria.
Assistive Technology Assessment
Prescribing a shower chair or an electric wheelchair are not the same job, we are experienced in basic and complex AT prescriptions. We trial equipment with you and write your report with recommendations.
Home Modifications Assessment
We have experience with all modifications, from ramps, bathroom access and doorway widths all. We assess the home against your participants goals, organise the construction, and sign off at the end of the build.
Manual Handling Assessments and Planning
When it comes to transfers, safety comes with a written plan. We assess all transfers, specify the right equipment, and provide educatation for every member of your support network.
Home and Living Assessments
Supported Independent Living and Specialist Disability Accommodation requests need support hours evidenced. We assess support hours, recommend staffing, and compare the housing options genuinely on the table.
BOT-3 Motor Proficiency Assessment
The BOT-3 is a standardised assessment of fine and gross motor skills for ages 4 to 25. It scores balance, coordination, dexterity and strength against age-matched norms, in numbers a school or a planner can use.

Speech Pathology assessments
Speech and Language Assessment
A speech and language assessment works out where a person's communication sits across language, literacy, social communication, speech sounds, fluency and voice. Our Speech Pathologists score the relevant areas with standardised tools and report what the results mean day to day.
Swallowing and Mealtime Assessments
Swallowing assessments checks how you eat and drink, done in your home environment. Our Speech Pathologists assess, then write a mealtime plan referencing the IDDSI framework.
Augmentative and alternative Communication
An AAC assessment works out which form of augmentative and alternative communication device will help a person communicate. We trial equipment, then write a report to assist funding a device.
Behaviour Support assessments and compliance reports
Recommended Treatment Action Plan
A Recommended Treatment Action Plan sets out the behaviour support work we recommend, in hours instead of dollars, so you can see what the funding buys. We review it at least every six months, and sooner where need, risk or funding changes.
Interim Behaviour Support Plan
An interim behaviour support plan gives a support team something safe to follow while the full assessment is done. Where a regulated restrictive practice is in use, the NDIS rules require it within four weeks of service commencement.
Functional Behaviour Assessment
A functional behaviour assessment works out why behaviours of concern are happening. We gather observation, interviews and behavioural data across settings, and what we find becomes the foundation of the behaviour support plan.
Comprehensive Behaviour Support Plan
A comprehensive behaviour support plan aims to improve a person's quality of life and reduce behaviours of concern. We write it for the support team who use it daily, with proactive strategies first and a reduction pathway on every restrictive practice.
Restrictive Practice Reporting & Authorisation
Regulated restrictive practices need authorisation, and this differs across NSW, TAS and QLD. We prepare the documentation, attend authorisation panels, and lodge through PRODA against your participants pathway.
End of Plan Review Report
An end of plan review evidences what a behaviour support plan achieved over its period, ready for the reassessment.
We review the funding used and remaining, and the NDIS Commission expects a review at least every twelve months.
Elite Physical Performance and Rehabilitation Testing
Strength and Power
Strength and power testing measures the force you push or pull with on a dynamometer, and how fast that force builds. We test both sides across the shoulder, elbow, wrist, spine, hip, knee and ankle, so progress at review is a number.
Movement Pattern Testing
Force plates record squat, lunge, jump, hop, landing, lifting and push up, which shows how load is shared between left and right during the movements that matter to the person.
Balance and Postural Control Testing
Force plates record centre of pressure travel and velocity during quiet stand and single leg balance, eyes open and eyes closed. It gives falls risk a figure a report can carry.

How much does an NDIS assessment cost?
Cost is usually the first question, so here are our standard assessment prices, current at September 2026. Where your plan includes funding for an assessment, the price is drawn from your plan.
Your service agreement sets out the exact price before we start. Home Modifications assessments are quoted individually. Prices for Behaviour Support assessments and Physiotherapy testing are confirmed when you enquire.

Tell us what decision the report has to support and when you need it. That is what determines which assessment is the right one.
We confirm scope before we start
We call you and we call the participant, to agree what the report has to cover and confirm the funding is there.
The assessment
We assess at home wherever we can, because that is where the barriers are. We use standardised tools where a score will carry weight, and observation where it will not.
Report writing and review
Functional capacity assessment reports are reviewed against a written checklist before release. Where more than one discipline contributed, each clinician is named in the report.
Sent, then explained
We book a follow-up after the report goes out, so the recommendations reach the people who have to act on them.
Confirming scope before we start
We agree what the report has to cover and confirm the funding is in place before the assessment is booked.
Can I use an NDIS plan for assessments?
Assessments are usually funded from Capacity Building, most often Improved Daily Living. Equipment recommendations are written against the Capital budget. We work with self-managed, plan-managed and NDIA-managed plans.
When a new assessment is needed:
A functional capacity assessment older than 12 months means a full new assessment, or older than 6 months where another provider wrote it. A full assessment is not always required, and the NDIS accepts a range of evidence.

Frequently Asked Questions
Who can refer for an assessment?
Support Coordinators, Plan Managers, general practitioners, other allied health professionals, and participants and families directly. Funding rules differ depending on the stream.
How do I know which assessment to ask for?
Tell us the decision the report has to support and we will tell you which assessment answers it. That conversation is free, and it is the cheapest point in the process to change your mind.
Do you assess in the home?
We assess at home wherever we can, because that is where the barriers are. Assessments also run in clinic, at school and in the community, depending on what the report has to evidence.
What if the participant needs more than one discipline?
It stays one referral. Optimum Health Solutions provides seven allied health disciplines, so a second assessment is arranged internally.
What if the case is too complex?
Tell us anyway. Multiple diagnoses, restrictive practices already in place, scripted high cost equipment and cases needing an advocate present are scoped as complex work, with more assessment and contact time built in.
